How do hyaluronic acid fillers work?

Hyaluronic acid is a substance found in the body's connective tissue. The enzyme hyaluronidase breaks it down by splitting the bonds between its sugar units [10].

Cheek filler — $800

Cheek filler adds volume to the midface, used when the upper cheek has flattened or lost fullness. In a controlled study of one volumizing HA gel (VYC-20L), approved in the US for age-related midface volume deficit, a median of about 2.0 mL total was used, and correction lasted a median of 24 months in the anteromedial cheek, 19 months in the zygomaticomalar area, and 15 months in the submalar area [4]. VYC-20L is not one of the fillers we stock, and those figures are specific to it. Within the Restylane family we stock, Restylane Contour and Restylane Lyft are FDA-approved for cheek augmentation and correction of midface contour deficiencies in patients over the age of 21 [12][13][15]. Smile line filler — $800

Smile lines (nasolabial folds) run from the sides of the nose toward the corners of the mouth, and filler here softens the crease. A meta-analysis of 18 randomized trials and 7 clinical trials found HA fillers effective for nasolabial fold correction, with mean improvement of 1.21 points on the Wrinkle Severity Rating Scale at 6 months; included trials ran 0.5 to 24 months [8]. That analysis pooled several different HA products, so what any one product does for you may differ. Within the families we stock, the RHA Collection [16] and Revanesse Versa [17] are FDA-approved for correction of moderate to severe facial wrinkles and folds such as nasolabial folds, as are Restylane Defyne [14] and Restylane Lyft [15]. Deep folds often relate to midface volume loss, so we may suggest treating the cheek too. Individual results vary.

Chin filler — $800

Chin filler adds projection to a chin that sits back relative to the profile, balancing it with the lips and jaw. In a randomized controlled trial of one HA filler (Restylane Defyne) in mild to moderate chin retrusion, 81% of treated participants improved at least one grade on a validated chin retrusion scale at week 12, and 74% were still improved at week 48 [5]. Results published for one product do not automatically transfer to another; we will tell you which product we are using and what is realistic for you. Participants received 1 to 2.9 mL at first treatment, and treatment-related adverse events were mild to moderate [5]. Restylane Defyne is in the Restylane family we stock, and is FDA-approved for augmentation of the chin region to improve the chin profile in patients over the age of 21 with mild to moderate chin retrusion [14]; Restylane Lyft carries a chin indication as well [15]. Jawline filler — $900

Jawline filler defines the jaw border and softens the prejowl area in front of the jowl. In a randomized study of one HA gel for jawline definition (VYC-25L), more than 70% of treated participants reported satisfaction with their lower face and jawline at month 6, with effectiveness sustained up to 12 months; measured prejowl depth improved 4.6 mm versus 2.5 mm in untreated controls. Injection site responses were numerically more common in women [6]. VYC-25L is not one of the fillers we stock, and those figures are specific to it. None of the fillers we stock is FDA-approved for the jawline: the approved indications of these products cover facial wrinkles and folds, the cheeks and midface, the chin and the temples [12][14][15][16][17]. Jawline filler here is therefore an off-label use of an FDA-approved filler: lawful and common in medicine, and something we tell you about and document in your consent before treating. Temple filler — $900

The temples are the hollows at the sides of the forehead, beside the eye; filling them softens the transition between forehead and cheek. In a study of 25 women given one HA filler injected between the superficial and deep temporal fascia at 1 mL per side, average hollow depth measured 4.33 mm before treatment, 0.71 mm at 3 months, and 1.62 mm at 6 months [7]. Those figures come from that single study and that one product; we will tell you which product we are using and what is realistic for you. The temple holds important blood vessels, so depth and technique matter [9]. What is facial balancing?

Fillers are used not only for wrinkles but to correct volume loss and facial disproportion [9]. Plans often combine filler with tox, which addresses movement rather than volume. Facial areas affect each other: a flat midface can deepen a smile line, and a recessed chin can make a jawline look softer than it is.

Which fillers do we use, and what happens at the appointment?

We use FDA-approved hyaluronic acid fillers, and we stock four families: the RHA Collection, Restylane, Revanesse Versa, and SkinVive (Juvéderm). Firmer, more cross-linked gels are generally chosen where structure matters and softer gels where movement matters [9]. We tell you which product we are using before we treat.

The areas on this page are not all approved areas for the same product. Restylane Contour is FDA-approved for cheek augmentation and correction of midface contour deficiencies, and for correction of temple hollowing, in patients over the age of 21 [12][13]. Restylane Lyft carries the same cheek and midface indication, plus indications for facial folds and wrinkles such as nasolabial folds and for the chin region [15]. Restylane Defyne is approved for facial wrinkles and folds such as nasolabial folds, and for augmentation of the chin region in mild to moderate chin retrusion, in patients over the age of 21 [14]. The RHA Collection [16] and Revanesse Versa [17] are approved for correction of moderate to severe facial wrinkles and folds such as nasolabial folds. None of them carries an FDA indication for the jawline, so jawline filler with the products we stock is off-label use: lawful and common in medicine, and something we tell you about and document in your consent before treating.

At your visit we review your history and medications, take photos, agree on the plan and price, then treat. For comfort we use topical numbing cream, lidocaine mixed into the filler, a dental or local nerve block, and ice; which of these we use depends on the technique being used, the areas being treated and your own comfort. Filler is placed with a needle or a blunt-tipped cannula. Appointments run 45 to 60 minutes. Judge the result at about two weeks, once swelling settles; if anything looks uneven, contact us and we will review it with you.

Aftercare and risks, including vascular occlusion

You get written aftercare instructions at your visit, and we go through them with you before you leave. Swelling, bruising, tenderness, lumps and bumps, and skin discoloration are recognized effects of filler; infection and biofilm are also described [3]. Filler can also move from where it was placed — see our guide to filler migration. In the trials above, most treatment-related events were mild to moderate [5][6]. Do not book filler days before a wedding or a photo.

The serious risk is vascular occlusion: filler blocking or compressing a blood vessel, cutting off blood supply and causing skin injury or, rarely, vision loss [1][9]. A 2024 review of a century of published cases found 365 new cases of vision loss after filler; the sites most often involved were the nose (40.6%), forehead (27.7%), and glabella (19.0%). Of 318 cases reporting outcomes, 6.0% recovered fully, 25.8% partially, and 68.2% not at all; the authors describe blindness and stroke from filler as rare [11]. Those percentages describe the mix of reported published cases — they are not your personal odds. Knowing facial anatomy and vascular danger zones is the main way this risk is reduced [9], and guidance stresses recognizing occlusion early and keeping emergency agents on hand [1][3].

Our reversal capability

We stock Hylenex, a brand of hyaluronidase [10]. It is FDA-approved as an adjuvant to increase dispersion and absorption of injected drugs, for subcutaneous fluid administration, and in urography [10]; dissolving cosmetic HA filler with it is off-label, supported by guidance describing low or moderate doses for non-emergency problems such as lumps or the Tyndall effect, and immediate high-dose treatment for emergencies such as vascular occlusion [2]. It carries its own risks. The prescribing information reports allergic reactions (hives or angioedema) in fewer than 0.1% of patients, and describes anaphylactic-like reactions as having occurred rarely. It is contraindicated in anyone with known hypersensitivity to hyaluronidase, and should not be injected into or around an infected or acutely inflamed area [10]. If your allergy history is unknown, we patch-test first, and epinephrine is kept on site.

Elective reversal is $275 per vial of hyaluronidase, explained in full on our filler dissolving page. We reassess at about two weeks and dissolve again only if it is still needed after that assessment; if it is, that is again $275 per vial. If we suspect a vascular occlusion, treatment begins immediately — that is an emergency, not a scheduled appointment.

Who is a good candidate?

Good candidates are adults in general good health who want a specific, describable change and understand that filler is temporary and that results differ between people. Filler does not treat skin laxity and does not do what surgery does. Dermal fillers are not recommended if you have active oral herpes, uncontrolled diabetes, blood-clotting problems, or lupus or another connective tissue disorder. We also do not treat over active infection or inflammation at the site. We do not perform any medical aesthetic treatments on pregnant or breastfeeding women.

Tell us about autoimmune conditions, blood thinners, prior injectables (where and when), planned dental work, and cold sores. If anything on your history needs a physician's input, we obtain that input before treating. Individual results vary.
