What is hyaluronic acid lip filler?

Hyaluronic acid is a substance that occurs naturally throughout the body and skin [5]. In an HA filler, it is formulated into a soft gel that an injector places into the lip tissue to add volume and shape.

HA fillers can temporarily enhance the lips and improve their shape, volume, and structure [5]. All hyaluronic acid soft-tissue fillers are temporary [5], so keeping a result means repeating treatment over time [1].

We stock four hyaluronic acid filler families: the RHA Collection, Restylane, Revanesse Versa, and SkinVive by Juvéderm. Which product goes into your lips is decided at your visit, and we will tell you which one we are using before we inject. Within the Restylane family we stock, Restylane Kysse carries an FDA approval for the lips: it is indicated for injection into the lips for lip augmentation and the correction of upper perioral rhytids in patients over the age of 21 [9]. RHA Redensity is FDA-approved for injection into the dermis and superficial dermis of the face for the correction of moderate to severe dynamic perioral rhytids — the lines around the mouth — in adults aged 22 years or older, not for lip augmentation [10]. Revanesse Versa is FDA-approved for injection into the mid to deep dermis for correction of moderate to severe facial wrinkles and folds, such as nasolabial folds, in adults 22 years of age or more [11]. Placing either of those two in the lips is an off-label use of an FDA-approved filler.

Full Lip or Half Lip — which one suits you?

Our lip appointments come in two sizes. Both are performed by the same provider using the same technique; the difference is how much product is placed. The price covers up to that amount of product, and how much is actually placed is judged at your visit — we respect your natural anatomy and lip integrity, and we never overfill the lips to use up product.

Full Lip ($750) covers up to one full syringe. It suits you if you want a clear change in volume, are starting from thin or flat lips, want to rebuild definition along the lip border, or are correcting an asymmetry that needs more product to balance.

Half Lip ($500) suits you if you are new to filler and want to start conservatively, want subtle hydration and shape rather than added size, are topping up an earlier treatment that has partly faded, or are treating one lip only. A Half Lip is the smaller of the two treatments and uses less product than a Full Lip. We will tell you exactly how much product your plan uses, and what it costs, before anything is injected.

What does "natural-looking" lip filler mean here?

Natural-looking means the lips read as part of your face rather than as a separate feature. In practice that means respecting your existing lip shape and proportions, and treating the lips in the context of the chin, cheeks, and jawline instead of in isolation.

Facial balancing sometimes means the honest answer is that lips are not the right starting point. If your concern is that your lower face looks unbalanced, chin or cheek support may address it more directly than more lip volume would. We will tell you that in the consultation.

What happens at a lip filler appointment?

You start with a consultation to talk through what you want, review your medical history and medications, and agree on a plan and a price before anything is injected. A $100 deposit is collected when you book a consultation and is credited toward your treatment. It is forfeited for a no-show or a late cancellation — cancellations and rebooking need more than 72 hours' notice.

For comfort we use topical numbing cream, lidocaine mixed into the filler, dental or local nerve blocks, and ice. Which of these we use depends on the technique being used, the areas being treated, and your own pain tolerance. Chilling the skin, a numbing ointment, and an injection of local anesthetic are all standard comfort measures for filler [4]. Injections are not painless, but they are usually easily tolerated [4]. The injection portion itself is short — each site takes only a few moments, and the full process of injecting, massaging, and assessing the result may run from about 15 minutes to an hour depending on how much is being treated [4].

You will see change immediately, but that first look includes swelling. Judge the result after your lips have settled, not on the day of treatment.

What is the swelling and bruising timeline?

Swelling, bruising, and firmness are the common responses after HA lip filler, and in clinical study they were mostly mild or moderate in severity [6]. Local injection-site reactions — swelling, bruising, pain, redness, and itching — are the most commonly reported adverse events in the published literature on HA lip fillers [7].

You may also notice an "overfilled" look, tenderness, temporary numbness or redness, or small palpable lumps at the injection sites [2]. Most of these conditions improve within hours to a few days, helped by topical ice and massage [2]; tenderness commonly lasts a day or two [4].

How long does lip filler last?

All hyaluronic acid soft-tissue fillers are temporary, lasting anywhere from six months to two years depending on the product and the area treated [5]. Maintenance treatments are needed to sustain the effect [5][1].

For lips specifically, ASDS describes lip enhancement as lasting up to six months [5]; a multicenter study of an HA lip filler found that more than half of participants (56.4%) maintained their treatment response at 12 months, with results lasting up to one year [6]. Across the wider literature, 71% to 93.2% of subjects showed a response on lip fullness scales [7].

Your own timeline depends on the product used, how much was placed, and your body. Individual results vary.

Lip filler aftercare

• Avoid intense physical activity for the first 24 to 48 hours to reduce swelling and bruising [2].
• Use ice on the area to settle early swelling [2][4].
• Avoid lipstick and pressure on the treated area immediately after your appointment [5].
• Contact us if something does not look or feel right, and seek urgent care immediately if you have visual disturbance along with pain or weakness on one side of your body [2].

What are the risks of lip filler?

Common side effects include swelling, bruising, bleeding, and lumps or bumps [5]. Less frequent problems include numbness, allergic reaction, infection, blisters, cysts, inflammatory reactions, and movement of filler material to another site [5]. Asymmetry, acne-like eruptions, and itchy rash are also on the reported list [3].

The serious risk is vascular: if filler enters a blood vessel rather than the intended tissue, it can obstruct blood flow. That can cause skin necrosis — ulceration or loss of skin — and, if the vessels supplying the eye are involved, vision loss [3]. Vascular occlusions can occur with dermal filler injection and can cause devastating outcomes [8]. Severe complications from dermal fillers are uncommon [3], but they are not zero. Knowledge of facial anatomy, careful technique, and the ability to recognize and treat a complication early are what reduce this risk.

One thing we want you to know before you decide:

We stock hyaluronidase. The product we use is Hylenex recombinant, a hyaluronidase (human recombinant) injection [12]. Hyaluronidase is an enzyme used in aesthetic medicine to dissolve hyaluronic acid–based fillers [8]. Having it on site means an HA result you dislike can usually be reduced or reversed, though how much changes depends on the product, how much is present, and technique — more than one session is sometimes needed. It is also the agent used when a vascular problem is suspected. If your allergy history is unknown, we patch-test first, and we keep epinephrine on site. Dissolving cosmetic filler is an off-label use of an FDA-approved medication — the Hylenex label does not list dissolving dermal filler among its indications [12]. Filler reversal is $275 per vial of hyaluronidase. We reassess at about two weeks, and if more dissolving is needed then, it is again $275 per vial. Who is a good candidate for lip filler?

Good candidates are adults in good general health who want a specific, describable change to lip volume, shape, or definition, and who understand the result is temporary and will need maintenance [5].

Filler is not recommended for people with active herpes (cold sores), uncontrolled diabetes, blood-clotting disorders, or connective tissue diseases such as lupus [5]. Tell us about blood thinners, recent illness, and any prior filler — including filler placed elsewhere.

We do not perform any medical aesthetic treatments on pregnant or breastfeeding women.

We may also recommend waiting or declining treatment if you have an event within two weeks, if your goal is not achievable with filler, or if we think another area of the face would serve you better. Individual results vary.
