Cheek filler at Bare.co Skin Health and Medical Spa is a hyaluronic acid (HA) gel placed deep in the midface to rebuild fullness over and around the cheekbone. Cheek augmentation and the correction of age-related midface contour deficiencies is an FDA-approved indication for Restylane Lyft with Lidocaine [13] and for Restylane Contour [14], both in patients over the age of 21, and Restylane is one of the filler families we stock. Why does the midface lose volume with age?

Two things change underneath the skin. The first is bone. The anterior maxilla — the bone immediately below the eye socket rim — shows marked resorption with age, most notably from the late 30s through the 60s [2].

The second is fat. Deep fat atrophy reduces midface projection, superficial fat changes alter the contour, and the dermis thins at the same time [3]. As the deep medial cheek fat loses volume and its supporting septa weaken, the skin and malar fat pad descend downward and forward [4]. So the change reads as flatness and shadow rather than as a line.

What does cheek filler actually do?

The malar fat pad is a triangular pad of fat, based at the smile line with its apex at the malar eminence — the high point of the cheekbone [4]. Cheek filler places gel at that apex, deep and close to bone, to restore projection where tissue used to sit.

Expanding the midface soft-tissue envelope effaces the nasolabial folds and reduces the filler needed to correct them directly [4].

For comfort we use topical numbing cream, lidocaine mixed into the filler, dental or local nerve blocks, and ice. Which of those we use depends on the technique being used, the area being treated, and your own comfort.

Should I treat my cheeks before my under-eyes?

Usually yes. Published guidance on infraorbital hollows recommends first addressing volume loss in the midface, because restoring volume there can improve the lid-cheek junction and reduce the filler that has to go directly into the under-eye area [5].

The under-eye is a less forgiving area to inject: in one injector's seven-year review of her own cannula-treated patients, 51% reported short-term swelling after infraorbital hollow treatment, and about 19% had longer-lasting or delayed-onset swelling when trained evaluators counted even minimal swelling [6]. Depending on the product used, infraorbital filler may be off-label — used in a location not stated in that product's label [6]. Off-label use is lawful and common in medicine, and we disclose it in your consent before treating.

How long does cheek filler last?

All hyaluronic acid soft-tissue fillers are temporary, lasting six months to two years depending on the filler [8]. In a two-year study of one volumizing HA gel (Juvéderm Voluma XC, VYC-20L), approved in the US for midface volumizing to correct age-related volume deficit, 92.8% of patients rated their cheek volume improved or much improved at 6 months and 79.0% at 2 years [1]. Those numbers belong to that one product, and it is not a product we stock. The hyaluronic acid fillers we stock are the RHA Collection, Restylane, Revanesse Versa, and SkinVive (Juvéderm), and we tell you which one we are using and what is realistic for you. If the product we choose for your cheeks is not approved for the midface, that is off-label use, and we tell you and document it in your consent before treating. Individual results vary.

How long does swelling last after cheek filler?

In a randomized cheek study of the same gel (VYC-20L), injection-site reactions were mostly mild or moderate and most resolved within two weeks; tenderness was reported for almost every treated cheek (91.7% with a cannula, 96.7% with a needle), and more patients had bruising on the needle-treated cheek only than on the cannula-treated cheek only (20.0% vs 8.3% of patients) [7]. Most early swelling and bruising improve within hours to a few days, helped by ice [9]. Do not book cheek filler in the two weeks before a wedding or a trip; judge the result at about two weeks, once swelling has settled, and contact us if anything looks uneven.

Malar edema is a persistent puffiness of the upper cheek. It is disfiguring, poorly tolerated, can persist for months, and responds minimally to treatment [4]. Filler placed superficially there can impede lymphatic drainage; placing it deep, on bone, reduces the risk [4].

Cheek filler aftercare

• Avoid intense physical activity for the first 24 to 48 hours [9].
• Use ice for early swelling, and avoid firm pressure on the treated area [8][9].
• Any change in your vision after filler is an emergency: seek care immediately, especially if it comes with pain or weakness on one side of your body [9][10].

Who is a good candidate for cheek filler?

Good candidates are adults in general good health whose concern is midface flatness or shadowing and who accept that filler is temporary. Cheek filler does not tighten loose skin; if laxity rather than lost volume is your issue, we will say so instead of treating.

Filler is not recommended for people with current oral herpes or a similar infection, uncontrolled diabetes, blood-clotting problems, or lupus or another connective tissue disorder [8], and we do not treat over active infection or inflammation. We do not perform any medical aesthetic treatments on pregnant or breastfeeding patients. Tell us about blood thinners, prior injectables, and planned dental work. A $100 deposit is collected when you book a consultation and is credited toward your treatment. It is forfeited for a no-show or a late cancellation — cancellations and rebooking need more than 72 hours' notice.

What are the risks of cheek filler, including vascular occlusion?

Swelling, bruising, bleeding, and lumps or bumps are common; numbness, allergic reactions, infection, blisters and cysts, inflammatory reactions, and migration of filler to another site are less frequent [8]. Asymmetry and over-correction are also reported [10].

The serious risk is vascular. Filler can be injected into an artery, or compress one from outside, cutting off blood supply [11]. That can cause skin necrosis — ulceration or loss of skin from disruption of blood flow [10] — and if the vessels supplying the eye are involved, vision loss [10]. Occlusion can progress rapidly to necrosis if it is not identified and treated quickly [11]. Severe complications from dermal fillers are uncommon [10], but they are not zero. Facial anatomy, careful technique, and recognizing a problem early are what reduce this risk.

We stock hyaluronidase; the product we use is Hylenex. It degrades hyaluronic acid and is the treatment for HA filler-induced vascular occlusion [11]. Dissolving cosmetic filler with it is off-label: hyaluronidase is FDA-approved as an adjuvant to increase the dispersion and absorption of injected drugs, for subcutaneous fluid administration, and in subcutaneous urography [12]. Its label reports allergic reactions (hives or angioedema) in fewer than 0.1% of patients and describes anaphylactic-like reactions as having occurred rarely [12]. If your allergy history is unknown, we patch-test first, and epinephrine is kept on site. Elective Filler Reversal is $275 per vial of hyaluronidase; we reassess at about two weeks, and if more dissolving is needed then, it is again $275 per vial.
